We are currently looking for an RN Case Manager.

Experienced clinical skills, organized, detail oriented, compassionate and a team player.
 
Responsibilities include admission of patients, coordinating patient plan of care, supervising skilled nursing and support staff.

Strong computer skills.

Workday: Monday-Friday
Hours: 8:00a.m.-5:00 p.m.
Rotating on call
